A sworn statement that a vehicle or boat was given as a gift.

Release of Lien
A lienholder’s certification that its lien on a vehicle or vessel is released.

Notice of Vehicle Sold Form
The seller’s notice to the MVD that a vehicle was sold or transferred and is no longer in their possession.
